Stephan is a Referral Coordination Associate – Social Care Networks at Anthos|Home. In this role, he supports the initial coordination process to help clients obtain housing navigation services by facilitating referrals, connecting individuals to community-based resources, collaborating with healthcare providers and partner organizations, and addressing health-related social needs that impact overall wellbeing and housing stability. 

Prior to joining Anthos|Home, Stephan gained experience in community health, care navigation, and member engagement through roles involving outreach, referral coordination, diligent searches, and re-engagement efforts for vulnerable populations. His background includes working with Medicaid and Medicare recipients, Health Home programs, and community-based initiatives focused on improving healthcare access, continuity of care, housing initiatives and social support services. He also conducted community, hospital, shelter, and home visits to support members with care planning, resource linkage, and ongoing engagement.

Stephan holds an Associate of Science degree in Community Health from CUNY Kingsborough Community College with a concentration of Health Education and Promotion, Stephan is currently pursuing his Bachelor’s of Science in Public Health at Brooklyn College.
